Pharos Innovations Announces First Recipient of National Award for Achievements in Chronic Care Programs and Research

is recognized for innovative research and programming for Iowa’s Medicaid population with chronic healthcare conditions
Northfield, Illinois, March 6, 2008 /PRNewswire/ — Today Pharos Innovations, a remote patient monitoring company specializing in the management of chronic care conditions, announced the first recipient of a new award honoring individuals for their breakthrough contributions in transforming the health of populations with chronic care conditions.
The 2008 Pharos Achievement Award was given to Dr. Bill Appelgate, Executive Director of the Iowa Chronic Care Consortium, for his leadership in developing a state-wide initiative for chronic care in Iowa, recently culminating in the development and validation of a congestive heart failure disease management program with Iowa’s Medicaid population. A letter of commendation from Iowa state senator, Jack Hatch (Democrat, Des Moines), was also presented to Dr. Appelgate at the award ceremony.

The efforts of the Iowa Chronic Care Consortium over the past five years have demonstrated major reductions in hospitalizations of patient’s with heart failure.
